2025 marks the fifth consecutive year of Medicare payment reductions to physical therapists, with the CMS conversion factor dropping to $32.35, a 2.83% cut that follows years of similar reductions. A typical PT visit, evaluation plus two to three therapeutic units, now reimburses approximately $90–$140 net from Medicare after MPPR reductions. Insurance plans frequently cap PT at 6–12 visits per year. Meanwhile, only 30% of patients complete their full course of care, and just 19–30% consistently do their home exercise programs.
